Patient Intake: The patient intake processes consist of identifying patients, performing initial outreach activities. Any follow up actions as a result of these activities are a part of this process.

Patient onboarding: This consists of one or many of the following onboarding and assessment processes. Patient onboarding includes processes to acquire patient's past medical record either through direct capture or via initial contact processes track planning and scheduling of care tasks. As part of initial contact and care tasks various assessment activities are completed. These include among other things, health status surveys questionnaire, demographics, social information, diagnosis, medications, results of past lab tests. After a patient is on-boarded for care the legal and contractual tasks are completed. All this forms a part of the Initial Outreach module.

Needs Assessment and care plan formulation: Needs assessment processes include measuring and monitoring risks to the patient. These initial risk assessments and scores help formulate care plans for the patient. These Initial assessments could be based on standard or custom questionnaires, and these assessments are used to calculate patient's risk score. This risk score is continuously monitored during the on-going care phase as well. These solutions help and support the objective of lowering the patient's risk score.

Treatment Monitoring and reassessment: The Ongoing Care modules support recording of information related to ongoing patient care. The Treatment monitoring processes monitor these tasks and creates alerts for subsequent interactions with the patients. Events monitored include patient visits, changes in medications, changes in diagnosis, changes in lab test results, modifications to treatment plans and progress in the health of the patient.
